Bacterial Infection!

It looks like you have a bacterial infection that has gone systemic. Your system could be under filtered or just dirty, so part of the solution will be to clean up your pond. What has worked well for me is a combination of Acriflavine, Proflavine Hemisulphate and salt. I would take your salt levels up to .06% and dose the others as recommended.
