Grow-out form please review

Below is the template I'm thinking we should use for the grow out. We have a chance to catalog the years growth of 150 koi. So in red will be the mandatory items. The rest doesn't have to be filled out but as a study on the effects of these shiro utsuris you might want to so everyone can see if the feeding habits, pond type etc make a difference. Please comment as this is the working draft. Thanks .

We will all be making our own threads in the grow-out forum. Please don't do that yet as I will make one when this form is finalized that you can copy from.

Koi 1 size when recieved: If Mystic is holding your koi ask for measurement.
Koi 2 size when recieved: If you only got one etc then disregard.
Koi 3 size when recieved: Size should be in inches

Koi 1 size between 6/1/08 and 7/1/08: Halfway mark of contest
Koi 2 size between 6/1/08 and 7/1/08: Photos will also be needed
Koi 3 size between 6/1/08 and 7/1/08:

Koi 1 size between 2/1/09 and 3/1/09: Help me out here. We could also end

I'd appreciate a full year and NOT ending it in November.
June, July, August, and September are the worst months for people in Florida.. I cut way back on my feeding then, and the koi don't mind. So having the Fall and Winter gives me a chance to grow em.

Under Pond Info, I think turnover rate and measured size (L x W x D) might help. Just my two cents worth.
